You can still do the job while leaving them in. Start by using the bristle attachment on your vacuum to clean any excess dust or debris on or between the grilles. If you managed to remove the registers, gently vacuum the vent opening. Be careful not to vacuum too far down into the ductwork, as that should be left to the professionals. 

After cleaning up all the loose dust and particles, focus on cleaning the registers. You can wash them or soak them in warm water and dish soap. If the registers were not removed, use a lightly moistened rag with dish soap and warm water to wipe them down. Be careful not to soak the rag to prevent water from dripping into the ductwork. Once the registers are clean and dry, put them back in place (if applicable). Regular maintenance like this improves the efficiency of your HVAC system and enhances indoor air quality.
